This policy explains what information Vetted Care collects through this website (vetted-care.com), how that information is used, and the choices visitors have. We have tried to write this in plain language. If anything is unclear, please reach out using the contact information at the bottom.

Important: do not submit Protected Health Information. This website is a public marketing and informational site for partnership inquiries. It is not a patient portal and is not HIPAA-secured for Protected Health Information (PHI). Please do not submit specific diagnoses, medication lists, medical record numbers, full dates of birth, Social Security numbers, insurance ID numbers, or other clinical details through the contact form. If you need to discuss clinical hospice care, please contact Lily Hospice directly at (248) 955-5100.

Who Vetted Care is, and what we do not do

Vetted Care is a service-disabled veteran-owned small business (SDVOSB) and a division of VETTED, a Michigan-based SDVOSB-certified federal contractor. Vetted Care is building a hospice care partnership with Lily Hospice — a Michigan-licensed, Medicare-certified, ACHC-accredited hospice provider — to deliver veteran-focused hospice care to Southeast Michigan veterans when the partnership is operational.

Vetted Care does not provide clinical, medical, or hospice services. Vetted Care is not a Medicare-certified hospice. Vetted Care is not licensed as a hospice under Michigan Public Health Code Part 214. Vetted Care does not directly accept clinical referrals, conduct hospice intake, or provide medical care. When the partnership is operational, all clinical hospice services will be delivered by Lily Hospice under its own CMS certification and Michigan license, with full administrative and financial management retained by Lily as required by 42 CFR 418.100(e).

What information we collect

Information you give us directly

When you submit our contact form, we collect the information you choose to provide: your name, email address, phone number (if you provide one), organization name (if you provide one), the topic of your inquiry, and the message you write to us. The form is designed for partnership inquiries, not clinical intake.

Information collected automatically

When you visit this site, our hosting provider (Netlify) records basic technical information in standard server logs: the date and time of your visit, the pages you requested, your approximate location based on IP address, the browser and device type you used, and the page you came from if applicable. This is standard log data that any website collects.

We also use a privacy-focused analytics service (Plausible) to understand which pages are most useful to visitors. Plausible does not use cookies, does not collect any personally identifiable information, does not track you across sites, and does not build a profile of you. It reports aggregate page-view counts and rough geographic regions only.

What we do not collect

We do not use third-party advertising cookies, tracking pixels, or behavioral retargeting on this site. We do not sell, rent, or trade your information. We do not embed social media tracking widgets. We do not collect Protected Health Information through this site. We do not accept clinical referrals through this site.

How we use your information

We use the information you submit through the contact form to respond to your partnership inquiry. If your inquiry relates to clinical hospice care for yourself or a loved one, we will direct you to Lily Hospice (or another Medicare-certified hospice) for clinical intake; we will not retain clinical details about a patient on Vetted Care systems.

We use server log data and aggregate analytics to maintain and improve the site, troubleshoot problems, and understand which content is most useful to visitors.

How we share your information

We share the information you submit only as needed to respond to you. If your inquiry is appropriately routed to Lily Hospice (for example, because you want to be notified when the partnership’s veteran-focused program launches), we will forward the relevant contact details to Lily Hospice. We do not share contact information with any third parties beyond our service providers (below) or as required by law.

We use a small number of service providers to operate the site: Netlify hosts the website and stores form submissions; Google provides the fonts displayed on the site; Plausible provides privacy-friendly analytics. These providers process information on our behalf under their own data protection commitments.

We do not sell your personal information.

How long we keep your information

Contact form submissions are retained in the inquiry system for as long as needed to respond to your inquiry and for our normal business record retention period after that. Server logs are retained for a short, rolling window for security and operational purposes.

How we protect your information

The site is delivered over an encrypted HTTPS connection. Contact form submissions are transmitted over encrypted channels and stored by our hosting provider with industry-standard protections. Access to submissions is limited to the members of the Vetted Care team who need it to respond to inquiries.

Because we are not HIPAA-secured for PHI on this site, we ask visitors not to submit clinical information through the contact form. If you do submit PHI inadvertently, we will delete it from our systems and refer you to Lily Hospice for any clinical follow-up. No system is perfectly secure; if we experience a security incident affecting your personal information, we will notify you in accordance with applicable law.

Your choices

  • You can contact Vetted Care by phone (248-794-9292) instead of submitting the form.
  • You can contact Lily Hospice directly (248-955-5100) for any clinical hospice inquiry.
  • You can ask Vetted Care what information we have about you and request a copy.
  • You can ask us to correct information that is wrong.
  • You can ask us to delete the information you submitted through the form.
  • You can ask us to stop contacting you.

Children’s privacy

This website is intended for adults. We do not knowingly collect information from children under 13. If you believe a child has submitted information through this site, please contact us and we will delete it.

Visitors outside the United States

This site is operated from the United States and the developing partnership’s planned services are provided in Southeast Michigan. If you are visiting from outside the United States, please be aware that your information will be processed in the United States, where data protection laws may differ from those in your country.

Changes to this policy

We may update this policy from time to time. When we do, we will update the “Last updated” date at the top. If we make a material change, we will take reasonable steps to notify visitors and any individuals whose information is materially affected.
